    • The forms available below are in .PDF format and require the Adobe Acrobat Reader program version 9 or above which can be freely downloaded from the Adobe website by clicking here.

      Mac users should ensure that they open and complete the document with the Adobe Reader that has been downloaded and not with "Preview", which is the default application for many Mac users.

      To file a STR, please follow the guidelines outlined below:

      Step 1:  Ensure you have downloaded the latest STR form and the STR supplementary form.  This may be done by carrying out any one of the following actions:
      a.  Left-click on the links below and wait for the .PDF forms to load. Then save the forms in a folder of your choice on your device; or
      b.  Right-click on the links below and choose ‘save target/link as’.  Then save the forms in a folder of your choice on your device.

      It is important to ensure that you complete the form that you have saved on your device.

      Step 2:  Once you start completing the forms offline, it is possible for you to save them so that you may continue working on the forms at a later time.  This should be done by clicking the ‘Save For Further Review’ button located at the end of the forms.

      Step 3When you have completed the STR form (and the supplementary form if needed) and are ready to submit it, you will need to click on the ‘Save For Final Submission’ button located at the end of the form so that the in-built programme will automatically check it.  Any mandatory fields which might have been omitted will be listed and the system will only allow you to save the form once these fields have been duly completed.

      Step 4:  To submit the STR form and any other documents which you intend to submit as attachments to the STR, please log-in or register on the FIAU’s online submission system by clicking here.
